Sendero offers care management services for members with long-term health conditions or complex care needs who could use extra support. The Care Management Team assesses your needs and provides close monitoring, education, and/or care coordination. Sendero’s Care Management team works with you, your family or care partner, and your doctor(s) to support and coordinate the services you need and to help you stay as healthy as possible.
